Reporting Error Revealed in Knee Pain Acupuncture Study, Catastrophe Avoided

On October 1st, 2014, the prestigious Journal of the American Medical Association (JAMA) published a study entitled “Acupuncture for Chronic Knee Pain: A Randomized Clinical Trial”[1]. It was a major study whose results stated that neither acupuncture nor laser acupuncture was useful to patients aged 50 and older with moderate-to-severe knee pain. Enter Dr. Arthur Fan…

In March of 2016 Dr. Fan and associates published a reevaluation of this study. It found serious flaws in its methodology and analysis. Not only that, but when properly evaluated, it appears that the study actually showed significant improvement in patients undergoing acupuncture and laser acupuncture in the clinical trial.

Dr. Fan is a consultant and long term researcher in Traditional Chinese Medicine who has been published in many of the world’s foremost medical journals. He has received numerous awards and commendations including a letter of thanks from the President himself. And thanks to him a grave reporting error was averted and an catastrophe avoided.

What is of interest to this writer is how such a mistake could have been made by one of the most respected, peer reviewed, medical publications in the world? When I experienced back issues years ago, I saw a orthopedic doctor, tried a chiropractor, physical therapy and pain medication. It was then I decided to give acupuncture a try. How fortunate that I found Dr. Fan. A very informed physician who was gentle and caring. After only a few treatments which included cupping, I was cured. To this day years later, I have not had any back issues. He is now treating me for acid reflux. I so recommend his treatment – just amazing.

Her vascular surgeon had told she needed to be on blood thinners for the rest of her life. Plus, she would have to wait for the clots to reobsorb back into her blood. Inside two weeks of treatment from Dr. Fan, the clots were gone. After my knee replacement the swelling was not going down. I was told that at my age it would take a much longer time frame. Three weeks of visits to Dr. Fan the swelling was completely reversed. We both whole heartedly recommend Dr. Fan.

But after seeing Dr. Fan and being treated with acupuncture and medicine he gave me, the symptoms have lighted and is decreasing daily. I usually get spasms where sometimes I can’t even move and/or walk straight. He is always concerned, he gives very good advice and is extremely skilled and helpful. His technique is amazing and I never feel whenever he puts the needles in. The cupping is also amazing- which most of the time are done by his great assisstant, April. One of my patient told me a story about the experience of his multiple benign tumors. He showed me the scars at his neck and abdomen from the operations.

He said he was a surgeon in China, in a big hospital in North. He found he had tumors and immediately used operation. The pathology study showed it is the neuro-fiber tumor. However, it recurred and became egg like tumors. A Chinese medicine doctor suggested him to try eggplant for 6 month. Then the tumors disappeared.

On November 21, 2011, Dr.Fan got a special call from a patient’s close friend, invited Dr.Fan to see a deep coma patient in INOVA Fairfax hospital neuroscience ICU ward. The visit was approved by that facility and its attending doctor.

The patient is a 67-years old gentleman who is a main minister of a local Indian Sikh temple. He got a hemorrhagic stroke in his brain stem area seven days ago when he did some home repairing.  The hematoma was large and he had an emergency surgery when he was sent to ER by an ambulance. When I saw him, he had no any reflection and no any reaction to strong stimulation. His respiration was irregular and had to use breath machine. He had fever, 102 degree F. He was under antibiotics and respiration stimulating medication.

Considering: Stroke, Bi Pattern(闭证).

I gave him An Gong Niu Huang pills(dissolve in warm water, once every 8 hours, i.g.), and did acupuncture for him.

Acupuncture points were: Ren Zhong, Yin Tang, Feng Chi, He Gu, Qu Chi, Ba Xie, Ba Feng, Zu San Li, Yong Quan. The stimulation was about one hour.

November 22, 2011: seemed have some effects. patient very occasionally move his leg slightly when I stimulated his Yong Quan points.

November 23, 2011: patient had more reaction to acupuncture, the frequency of moving his leg, fingers, head, slight more than one day ago.

November 24, 2011: condition was same as before. The tube from skull to test ICP removed.

November 25, 2011(5th treatment): patient was still in deep coma, responding to acupuncture stimulation more often, seem every 5-10 stimulation he could have a responding.  Stopping use antibiotics and respiration stimulating medication. The iv tube to central vein was removed and only used iv tube in arm vein.

November 26, 2011(6th treatment): patient was moved to Intermediate Unit from ICU. The condition was not good as yesterday. The doctor there discussed patient’s condition with his family members, suggested that this patient was still in very critical condition and his future was not so optimistic. Patient’s family members seemed considering give-up.

I did not give patient more treatments since then.

During six days I involved in his treatments, I feel Chinese medicine, included in An Gong Niu Huang Pills and acupuncture did show some effects, such as some responds to pain stimulation(relection recovered a little bit). However, this patient’s condition is too severe, he didn’t recover from the deep coma.

Although this invitation was by patient’s family and close friends, the facility and doctor in ICU did allow the doctor or practioner in Chinese medicine /acupuncture to see and treat patient. This is much open than a few years ago.

I remember, I saw a coma patient in Johns Hopkins Hospital two years ago, that facility did not allow patient to use Chinese herbal medicine, which needs special approval by hospital administration (the procedure would take half-year).

Infant, baby, ChildI often got call and asked me–

 “Do you treat Children’s condition?”, or

 “How old the child could be got acupuncture?” or

 “Do you think it is safe doing acupuncture in baby?”

I always reply–children at any age could be got acupuncture, acupuncture is very safe in children, and most important–acupuncture is more effective in children than adults due to children’s physiological characteristics.

For acupuncture in children, we have more confidence because:

(1) Dr.Fan had special apprenticeship with Dr.Su, Shang-yi,when he was a TCM student during 1981-1986 in Nanjing University of Chinese Medicine. Dr.Su was very famous in pediatric acupuncture, and saved many children’s life, esp. for those in danger condition stayed ICU of Nanjing Municiple Hospital at that time, such as Respiratory failure or Circulation failure.

(2) Dr.Fan has young patients almost everyday, he has accumulated much experience.

Recently, Dr.Fan treated a 8 weeks old baby, Alexandria.Z. with Accide Reflux and Laryngomalacia. Before the treatment, baby had milk spill out of mouth, acide reflux, which caused hard of breath, sometime, breath stopping with face pulple, baby bowle movement also had difficulty, she push stool or gas a lot, which also cause milk spill out of mouth. Only after 2 times’ acupuncture, the baby has improved significantly. No longer milk spill out of mouth, no longer strugle with the breath, and very calm and no more strugle with pushing the gas or stool.

Here, we show a picture of the baby A.Z.with Dr.Fan.She is very calm.

Baby A.Z. eight weeks old is my youngest patient, our oldest patient is 92 years old, with lower back pain.

My daughter, J……, just started a trial vegan diet and I wanted to know your philosophy on long term effects of a vegan diet. I read somewhere where they said too much of one type of food is not necessarily good. How does this apply to vegan food and how would you modify this?

I am not entirely sold on the idea of a vegan diet. I believe in consuming a moderate diet with lean meat and vegetables, however, I will support any of my family members trying an alternative food diet.

What is your philosophy on dieting and specifically, vegan diet?

Hi,Mr.R.S.,

Basically, the philosophy of Chinese medicine is “going along the meddle line”. Everything which goes to extreme is not so good. For the healthy issue, our philosophy is eating a balanced food, using vegetables, fruits and animal foods.  Based on personal favorite, someone may eat a little bit more on specific food category. i.e. more vegetables or animal foods.

However, if the person has some weakness in his/her constitution(physical pattern), we may advice that person using special food, included in animal foods depend on people’s condition.

As a healthcare provider, Chinese medicine doctor focus on patient’s actual need to adjust food or add herbs,acupuncture,etc, the main point is letting patient’s condition back to balanced status. We don’t like to go an extreme route, which may only depend on a political sense. To some people, vegan food may be good, some not at all–depend on each person’s constitution.

Everyday, I answer many questions from visited patients or online inquiries, such as:

For some chronic or difficult-to-treat conditions, such Dystonia, or Parkinson Disease, or diabetes, or a mix of many conditions,

“How many sessions’ acupuncture could cure my condition?”

“How much percentage of cure rate of your treatment?”

“I have infertility, how many sessions’ acupuncture could help me get pregnant?”

And, “If you say you could cure it, I will see you.”

Actually, many conditions or illness,disorders are chronic conditions, many of them are difficult-to-treat also, and patients may use conventional medicine for years too.

Frankly, I say “we could not cure most of them”,  in 8 sessions, even in 16 sessions’acupuncture; Or in one month herbal tea treatment. Many of them need long-term management.

We do see some magic results in some cases; we did cure some patients in a very short-term treatments. In a few cases, we even cure the patients’ condition, which they had that for years, with only one or two sessions’ acupuncture or in one day’s herbal tea treatment!

We did see pregnant only after one acupuncture treatment in a few patients with infertility. We did stop the dystonia in one week.

However, for most of the cases, we could improve them; some patients also did get disappointed with our treatments, because their aims did not become the real in one or two month, or other short period of time.

We do tell patients who have difficult-to- treat illness or disorders, they will see some of improvements over the time, some illness or disorders could not be cured in 100% of patients:  some may get “cure”(this is clinically cure, not absolute cure as patient hoped, sometime may get relapse if patients get some trigger), some get improved, some get slight better(or other condition get better, for example, overall condition better), some no result, some worse (please note, the illness, or disorder will get worse under some conditions by itself, not related to acupuncture or Chinese herbs at all).

We could not imagine how many percentage of the patients get “cure”.  For example, we have over 40 patients get pregnant in recent 4 years. We may say about 40-50% of patients (if they had 2 month or more serious treatments) get pregnant!

However, this is a very rough answer. Why? the condition is very complicated.

In these patients who seek acupuncture/herbs for fertility, patients had different causes of infertility, such as FSH high, POCS, ovulation issue, progesterone issue, tube issue, husband sperm issue, etc; many patients had medications, some of them did acupuncture alone, some of them had Chinese herbs too; some of them followed regular treatment strategy, some of them didn’t follow our strategy very well(due to work, due to personal issue, due to something else–subjective or objective reasons); some of them had IVF, IUI etc(with acupuncture or herbs). We could not tell you very detail in a statistic work.

However, it does work!

For acupuncture and Chinese herbology, to many difficult-to-treat illness or disorders, it works! at least it improves patients’ condition in most of the cases.

Importantly, acupuncture basally has little adverse effect. Chinese herbology also has very less side effect if patients see a trained Chinese medicine doctor.

For most of patients with chronic or difficult-to-treat conditions, no.one thing is adjusting the aim based on understanding their own condition well, don’t see a Chinese medicine doctor – he or she promised you–you will get a cure in several treatments(except for some pain condition). Most of time, patients will need more longer treatments.

Be patient.

For the cost-effectiveness, don’t see a provider who promise you have a good result but you pay very little.Most of well-trained, experienced Chinese medicine doctors offer patients a reasonable, affordable fee schedule. For better result, don’t see a provider who had less training and over low fee.

Online literature is very few in Chinese medicine treating Parkinson disease. We published two papers in this scope in Chinese language in 1990s.

After I settle down in America, I did not report my clinical experience in this field.

Recently, I have a successful case in Parkinson disease, using scalp acupuncture and traditional systemic acupuncture, as well as Chinese herbal tea, now patient recovered 70%.  He said he was embarrassed before, now he has confidence to see other people, because in most of time, he no longer shakes.

K. N. , an eleven years old boy, was recovered from his half year extreme fatigue, low fever which was from mono, last week.

Six month ago, he was very sick, the diagnosis was Mono, a virus illness, and had some treatments from his conventional medical doctor.  Since then, he always feels very tired, some time has fever.  He had extreme fatigue, had to sleep 12 to 18 hours a day, so he missed his school over one and half month.

When he came to see me, his face was pale, tongue and lips were very red, complexion very tired, and eyes showed no-spirit.  Pulse slight quick. Temperature 99.1 degree F.

We diagnosed him as blood- heat syndrome, used Chinese herbs and acupuncture to treat him.  In second day, he backed to school. Yesterday he had the third visit. His mom and he were very happy, everything is stable, no longer sleep so long hours, he said he has “too much energy” and only need sleep 7 to 8 hours.

Our experience is, the syndrome post mono, basically is extreme fatigue or low appetite, using acupuncture or Chinese medicine is very easy to get recover. This case is my no.4 case of Mono, all of these patients got recover in one or two weeks.

His brother J.N.  saw me for his chronic cough, due to allergy and asthma issue, after the first acupuncture treatment, his whistle sound in his lungs disappeared.

Sometime, patient, who has tried to conceive, asked me the question–“Dr.Fan, for infertility treatments, do you think it is necessary to use different acupuncture points, based on different period phases?”

Similar question we could ask–“Dr.Fan, using Chinese herbology, what do you comment to using different herbs according to women period phases?”

My answer is:

TCM treatments based on period phases, either acupuncture or Chinese herbology in Chinese Gynecology, actually was based on my two teachers, Dr.Xia Gui Cheng (some books published in western countries in recent years  in English language may reflect Dr. Xia’s experiences and theories) and Dr. Sun Ning Quan (he died in early of 1990s), who clearly mentioned the relationship between women period phases and Yin-Yang cycle in 1970s.  They clearly mentioned using different Chinese herbs/ formula to treat the illness/disorders which are related to women’s period cycle, or adjust the irregular period to reach the aim of conceiving.  Before them, almost all Chinese medicine doctors use traditional Pattern differeciation (Bian Zheng Lun Zhi) to treat female patients.

So, we could have two different ways / strategies to handle same problem, i.e.infertility–either traditional Bian Zheng Lun Zhi or according to period phases(actually another style Bian Zheng Lun Zhi).  For teaching in class or writing / publishing a book, the practitioners (TCM doctors, teachers) may prefer to using different methods to treat patients in different period phases.  Such as in period flow time, using certain methods; in estrogen phase, using certain methods; during ovulation, using certain methods; and during progestrone phase, using certain methods.

In theory, we prefer that, but we need to consider the real practice condition.  This rule/method could adjust according to fit different conditions.

Using only one set of treatment strategy may not fit to everyone, esp. to patient who has some experience from other practitioners’ office or reading some books. However, sometime, we have to use it, of course, still need modify accordingly.

Talking acupuncture first.

For fertility issue, we, indeed,could do different “menu” for different phases of period, however, some patients are not like certain positions or some points.  I mean we face some challenge sometime. So we have to use equievlant points in a patient’s confortable position.

Second, we find it is not uncommon–when patients come to see me, sometime they forget the exact days in their period cycle. When I ask– “today, what day is in your period cycle?” Patients may reply–” Oop! I need ask my husband”, or ” I need look for my calender” (today I met two!).

And it is very common–” I have/had a travelling”, “I am/was busy…..”, “I am sorry to miss the appointments”.  So it seems we could not do something in real time in some patients.

Sometime, doing acupuncture according to patients’ period cycle seems very difficult.

So, based on 25 years of my clinical experience, I feel basing on traditional strategy sometime is also a good option.  I use one bigger acupuncture formula to adjust according to specific condition, which includes a few things (here, we use conventional words / terms, although we do consider it in TCM theory and terms, the original thinking style):

1. Adjusting the energy level to handle the stress or mental issue, which affects gynecological function;

2. Adjusting hypothalamus and pitutary function which is upper center for ovaries and uterus functions;

3. Adjusting the local function of ovaries and uterus functions, and circulation of pelvic area.

Total how many needles for this formula? 25-30 per time.

Each time, we basically do not want to adjust the formula totally, may be adjusting 3-5 needles based on new condition or phases issue.  One reason is about half of our patients don’t like to use less needles (“why last time used 25, today use 21?”)–they paid, so they want to use more, at least same amount needles; And, explanation takes too much time. However, we do adjust if different patterns, or patients’ different reactions occur.

The results from our center for fertility is good, over 40 ladies got pregnant, either acupuncture alone or with IVF, IUI since 2007.

Talking about Chinese herbology.

In China, the herbology for infertility treatments has been a main therapy since 2000 years ago.  Before my teachers, “the great master”, Dr. Xia Gui Cheng (who helped Chinese president Hu Jing Tao to get his grandchild) and his former colleague Dr. Sun Ning Quan (he was the first one who mentioned using TCM herbs to adjust patients’ period based on patient’ s period phases), in history, TCM gynecologists almost all using single method to treat infertility, such as adjusting Live Qi, Regulating the Blood, or Tonifying the Kidney and Spleen…… Since my teachers, the rule has been changed–adjusting women’s period according to period phases.  It is sure, I am their “good student”.

However, in United States, the culture is very different.  Using herbs meet some challenges in present time.

No.1 issue is patients seem not like the tastes, and using too much time to cook; and it is not uncommon–forgot to take in time.  “Too busy” is one of their stories.

No.2. FDA still treats herbs as food, so any side or adverse effect is not allowed for herbs.  And, conventional MDs are not welcome new remedies which may have a competition with them–they have no training in herbology too, so they might always give negative advices to patients for herbs.

So, 2/3 patients could not use herbal tea regularly, 1/2 patients could not use herbal pills in time.

For getting more positive results, I have been working on more herbal application combining with acupuncture.

Following the Big Tao(Dao), Making Everything Real Simple” (Da Dao Zhi Jian).  Using one set of treatment plan to modify in fertility treatment is my current main method.

I heard famous TCM gynecologist Dr. Bei Run Fu in California also has simplified his treatment strategies. In China(15 years ago), he prefered the Dr.Xia’s theory and published many papers; but when he came in USA, he gradually comes back to the traditional Bian Zheng Lun Zhi, to use one strategy to modify the formula in fertility Chinese herbology practice today.

“Dr.Arthur Fan, Could You Tell Me by Taking My Pulse–If I Already got Pregnant?”

Many patients or friends of mine have asked me such a question,my answer is “NO!”

Surprise?

Maybe, a little bit?

Some Chinese Medicine doctors/practitioners may tell their patients–they could tell if the female gets pregnant or not–by Taking patient’s pulse only.  To the patients or people who don’t know Chinese medicine, it seems really very magic! And this may attract some patients to see such providers.

However, I have to tell you the truth–We COULD NOT tell you if the woman already got pregnant or not, only according to the Pulse diagnosis. 

The condition is very complicated.

Indeed, when a woman gets pregnant, her pulse should be stronger and bigger than before, we call this style pulse is “slippery Pulse”(Hua Mai).

But, if a female has slippery Pulse, we could not say–“it is sure you got pregnant”. The slippery pulse only reflects that the body’s Yang Qi is over strong or at moving.

When a female gets pregnant, her Yang Qi is stronger and stronger day after day due to her hormone changes during the pregnancy, such as HCG’s increasing significantly over the time.

However, when a female does not have pregnancy, such as getting her period, or getting ovulation, or she just had a sex, or just after an exercise; sometime, we also could find she has a slippery Pulse (or similar pulse)!

When a female takes some special medications/drugs, her pulse also becomes slippery!!  For example, using prednisone or Progesterone.

When a female has hypertension, or headache, her pulse may become slippery too!!!

Only, when a female during her good age for pregnant (between her first menses to menopause), had sex with male (or IUI,IVF), then her period stopped over one week, her pulse becomes slippery, we could say this female has a bigger possibility–she may get pregnant. 

However, the golden criterion is her HCG hormone significantly increasing, not the Slippery pulse.

So, if a provider tell you–“you get pregnant”–only after pulse taking and did not ask you the period condition, medication and other related issues, you should question him/her–“Did you had good training in Chinese gynaecology?”

Mr. S.B. , a 59 years old government officer who has Chronic Lymphocytic Leukemia(CLL) over two years. After our Chinese medicine treatment (acupuncture-once a week- plus herbal tea, 3 pack- herbs a week) for 8 months, his condition has been under the control and his blood count improved a lot.

He has no symptom, but his blood count has been abnormal for a few years (he has seen doctors in Georgetwon University Hospital for many years. He did not use chemotherapy for CLL yet).

His Complete Blood Counts as follow:

in 08/19/2009(one month before our treatment): WBC 14.9 (normal range: 4-10.8); Lymphocyte:10.30(normal range:0.6-4.9), 69.2%(normal range: 15-45%);

After our treatments:

01/20/2010(two months after our treatments): WBC: 10.90; Lymphocyte: 7.50, 69.1%;

07//28/2010(8 months after our treatments): WBC: 8.30 (within the normal range); Lymphocyte: 5.10(almost reach the normal range upper level), 61.6% (still in abnormal).
